If a parent/guardian would like place their student's name on the waiting list for a Pre-K Program, they should complete a Post-Lottery Pre-K Registration Form. To access the form, click the link at the bottom of the column on the left side of this page. The form will require the following information: student’s name and DOB, parent/guardian’s name, address, email , phone number and list the name of the school.

In chronological order, the Early Learning Team will contact the parent/guardian via email or phone for placement in the Pre-K program. Upon receiving placement, the parent/guardian will complete the Online Registration Application at www.clayton.k12.ga.us. To ensure immediate enrollment into the program, the parent/guardian should upload the required documents listed below.

Documents required to complete enrollment:

* Pre-K Acceptance Letter (via email)
* Proof of Age (i.e. original birth certificate, passport, etc.)
* Parent’s Government Issued Photo ID (custody/guardianship documents)
* Two Current Proofs of Residency (last 30 days)
    (mortgage/rental agreement, utility bills)
* Immunization Record (Form 3231) or Waiver
* Eye, Ear, Nutrition, and Dental Examination (Form 3300)
* Social Security Card or Waiver (optional, but strongly encouraged)

Upon completion of the online registration enrollment application, parents will receive email notification providing additional enrollment information.
